This consultation includes an on-site visit to the student&#8217;s school district from the Deaf and Hard of Hearing\u00a0Outreach Transition Coordinator.<\/p>\n<p>The Transition Coordinator will assess the student\u2019s transition-related needs through:<\/p>\n<ul>\n<li>Interviews with the student, family and educational staff<\/li>\n<li>Informal assessment of readiness skills<\/li>\n<li>Interest inventories (preferences, experiences, goals)<\/li>\n<\/ul>\n<p>Following the consultation, a written summary will be provided.
